NEW STANDARD SURVIVORSHIP CARE PLAN Standard: The cancer committee, or other appropriate leadership body, develops a process to implement and monitor dissemination of a comprehensive care summary and follow-up plan to cancer patients completing cancer treatment. The process is monitored, evaluated, and presented at least annually to the cancer committee, or other appropriate leadership body, and documented in minutes. DEFINITION AND REQUIREMENTS The Institute of Medicine and National Research Council 2006 report titled From Cancer Patient to Cancer Survivor: Lost in Transition recommended that cancer patients completing first course treatment, including treatment for recurrences, should be “provided with a comprehensive care summary and follow-up plan that is clearly and effectively explained.” The recommendation suggested that these plans would help cancer survivors who may otherwise get lost in the transitions from the care they received during treatment through the phases of their life or stages of their disease course. The purpose of this standard is to have cancer programs develop and implement a process to monitor the dissemination of a Survivorship Care Plan as a part of the standard care of the cancer patient. Process requirements: (a) A Survivorship Care Plan should be prepared by the principal provider(s) who coordinated the cancer patient’s oncology treatment. (b) The Survivorship Care Plan should be given to the cancer patient upon completion of their first course treatment, including treatment of recurrences. (c) The Survivorship Care Plan should contain a record of care received, important disease characteristics, and a written follow-up care plan incorporating available evidence-based standards of care. The Institute of Medicine’s Cancer Survivorship Care Planning Fact Sheet includes minimum plan standards. Additional resources are available to assist programs with the development of these tools including care planning templates. Care planning templates are available from e.g. the American Society of Clinical Oncology, American Cancer Society, National Coalition of Cancer Survivorship, and the Lance Armstrong Foundation. SPECIFICATIONS BY CATEGORY All cancer programs must fulfill this standard as written. DOCUMENTATION The facility completes the Survey Application Record (SAR).
